Barium Meal Follow Through Radiology

Barium Meal Follow Through can be done through three methods:
  • Single Contrast: Only Barium Sulphate is Given.
  • Double Contrast: Barium Sulphate + gas producing material like carbex which arise co2 gas is given to produce double contrast effect.
  • Penumocolon Method: when the barium sulphate is reach up-to the ileocecal junction then a gas producing  material is injected from rectum through catheter to produce double contrast effect. 

Indication Of Barium Meal Follow Through

  • Pain, Gastric pain
  • Diarrhoea
  • Gastrointestinal Bleeding / enema
  • Partial obstruction in Intestine
  • Malabsorption
  • Abdominal Mass
  • Failed Barium Meal Follow Through Procedure

Contraindication Of Barium Meal Follow Through

  • Complete Large bowl obstruction
  • Suspected Perforation ( then barium sulphate is not used / water soluble contrast media is used )

Contrast Media used In Barium Meal Follow Through Procedure

  1. Barium EZ-HD 100% W/V      Baso4 = 300ml
  2. Barium Meal & Barium Sulphate is done in a combination then just after 150 ml of EZ-HD 100% is given
  3. In Case of child patient 3-4 ml Baso4/kg 
  4. If the barium sulphate is contraindicated then water soluble Non- ionic contrast media is given.

Preliminary Film

  • To check the bowl preparation
  • Full length abdomen image is taken in supine position
  • Procedure is not done at that time next appointment is given to the patient.

Patient Preparation

  • 2-3 days before low residual diet is given to patient
  • Before 6 hr of Barium Meal Follow Through Procedure no liquid & solid food is not given.
  • 30 min before metchlopromite 20mg is given to decrease the transit  time of stomach to intestine.
If the bowl preparation is correct then BaSO4 is given to the patient in the supine right side position orally.

  • Tell patient to ley down on the right side because the barium is travel fast from stomach to intestine.

In 1st hr Of Barium Meal Follow Through Procedure 

  • First take a image of abdomen in every 10 min then the image is taken in every 30 min all images are taken in prone condition. prone condition is used to increase the pressure to separate the bowl loop.
  • When Barium is reach up to the ileocecal junction then image is taken in supine condition.
  • In this case compression paddle is used to separate the bowl loops.
  • Different image is taken in different angle i.e oblique image is taken of abdomen.
  • Head down image is taken.

After Case In Barium Meal Follow Through Procedure

  • Tell patient the bowl motion is white for some days.
  • to stop the impact of barium tell patient to drink more and more water.

Complication In Barium Meal Follow Through Procedure

  • Suspected Perforation 
  • Partial obstruction to complete obstruction
  • Barium Apendetitis.
